PRIVACY POLICY
In accordance with changes to the Privacy Act 1988 the Privacy Policy of this Practice has been updated to comply with the new Australian Privacy Principles which come into effect in March 2014. A full copy of our Privacy Policy is available via the link below or a hard copy can be obtained from Reception.
This new Privacy Policy contains information regarding
1. The kinds of information the Practice collects and holds;
2. How the Practice collects, hold and uses personal information;
3. How you may access your personal information;
4. How you can correct the personal information we hold;
5. How you may complain if a breach of the Australian Privacy Principles occurs; and,
6. Whether the Practice engages with overseas recipients.
It is the policy of this Practice that any requests for access or release of information, by a patient or a third party, are in writing. In some cases it will be required that the identity of the third party can be verified. Any requests for information from a third party will be verified with the patient concerned before any information is released. Only information that is deemed appropriate and relevant by the doctor will be released unless bound by law or other legislative process.
If you wish to access your records we would appreciate this request also being put in writing. We would then prefer that you make an appointment to access the records with the doctor to ensure the contents are understood and to safeguard the privacy and confidentiality of others who may have contributed information to your record. Please note such a visit would not receive a Medicare rebate.
All requests will be dealt with within 30 days of being received. Please advise us if the matter is more urgent.
